The Aboriginal outreach worker’s role is to encourage and support Aboriginal and Torres Strait Islander peoples with chronic disease to access health services and help to ensure that services are culturally competent.
We aim to improve access to appropriate health care through care coordination and provision of supplementary services for eligible Aboriginal and Torres Strait Islander people with chronic disease.
Aboriginal outreach workers carry out non-clinical tasks, e.g. helping clients to travel to their medical appointments including GP care planning, follow-up care, specialist services and community pharmacies.
