DESCRIPTION:Available for Aboriginal and/or Torres Strait Islander individuals 50 years and over.\nThe CHSP program provides small amounts of entry-level support to assist older Aboriginal and/or Torres Strait Islander peoples aged 50 years and over and HACC-PYP supports community of 50 years and younger. Social Support Group provides an opportunity for clients to attend and participate in social interactions which are conducted away from the client’s home.
