Aboriginal Health Promotion & Chronic Care Support Worker is a non-clinical service and plays a vital role in supporting people with advocacy, supporting clients to access appropriate health care services, empowering clients to take control of their own health & well-being.
Aboriginal Health Promotion & Chronic Care Support Worker supports clients with Chronic Health issues to manage their own self-care, improve social and relationship skills and achieve broader quality of life via physical health, social connectedness, housing, education and employment.
The core function of this position is to work with the Upper Hume Aboriginal and Torres Strait Islander community in offering education and support to the people at risk of developing or diagnosed with chronic illness to access appropriate health care that is culturally respectful and addresses aspects of health including prevention and treatment.
Eligibility
To be eligible for Aboriginal Health Promotion & Chronic Care Support services a person must:
- Be registered with the AHPACCSW program
- Have a chronic illness or at risk of developing a chronic illness.
- Must be of Aboriginal & Torres Strait Islander decent.
- Be a carer / family member of an Aboriginal/Torres Strait Islander person with a chronic illness
- GP referral or GP management plan
